The plugin looks at the extension for recognizion of a pdf file rather the the file header. I had apache with mod_rewrite on. for example: /doc/<name>/ was converted in /doc/foo.pdf The result was just a text print of the pdf file in the browser. the extension must be in the url (.pdf) for the plugin to load so: /doc/foo.pdf without mod_rewrite worked fine. Maybe it saves someone time :)
