note 82995 added to function.str-getcsv

RFC 4180 which deals with CSVs states the escape character is supposed to be a double quotation mark: (page 2) 7. If double-quotes are used to enclose fields, then a double-quote appearing inside a field must be escaped by preceding it with another double quote.
