You're getting the error because you have the arguments to mysql_query backwards. You have
this:
<?php
$add_topic_res = mysql_query($mysql, $add_topic_sql)
or die(mysql_error($mysql));
?>
You SHOULD have this:
<?php
$add_topic_res = mysql_query($add_topic_sql, $mysql)
or die(mysql_error($mysql));
?>
