I do not experience 2 of your issues with MySQL 5.0.77 and PHP 5.2.6:
1) I can execute a statement like 'SELECT :num WHERE id=:num AND x=:num' with
array('num' => $value). All placeholders will be replaced.
2) MySQL's query cache is used no matter if I use PDO::query or execute on a prepared
PDOStatement.
