Most likely the issue in question is the pcre.backtrack-limit
I've run into the same problem and using ini_set("pcre.backtrack_limit",1000000) to
bump the limit up some resolved the problem.
Of course, preg_replace could fail for other reasons also. After calling preg_replace you should
check preg_last_error() for error codes.
