Iuri Jacob noted below that he could only access a remote server using the CGI Method. I also had
issues connecting a remote server until I tried appending the default port number (1433) top the
server string. eg;
$server = 'domain.com';
...did not connect, but the following did...
$server = 'domain.com:1433';
