Example #6 is a big vague on which lines are misusing the placeholder.
This is correct usage:

$stmt = $dbh->prepare("SELECT * FROM REGISTRY where name LIKE '%?%'");
$stmt->execute(array($_GET['name']));
?>
This is NOT correct:

$stmt = $dbh->prepare("SELECT * FROM REGISTRY where name LIKE '%?%'");
$stmt->execute(array($_GET['name']));
?>
