During my testing, I found that using a link for a css file caused flush() to not work as expected:
<link rel="stylesheet" type="text/css" href="test.css" />
I was able to resolve the flush() issues by reading the css file directly into my html:
<style>
<?include("test.css");?>
</style>
These results were found while using firefox, however it may also apply to other browsers.
