Under FreeBSD 4.1 with PHP4 using the ports collection.
Ports almost gets it 100%. You need to change the tdsver to 4.2 instead of 7.
In '/usr/ports/databases/freetds/Makefile' make the following change:
- --with-tdsver=7 \
+ --with-tdsver=4.2 \
