note 17628 added to function.xslt-process

This documentation does not reflect the processor for 4.0.6. This only works on the default transformer, and will not allow you to provide your own. including the result from xslt_create() only causes problems. This should be xslt_process(xslFile, xmlFile, result) - the function will not work if you pass a transformer handle to it.
