As of 4.1.1 you can't call ImagePSExtendFont more than once for the same font resource id. In
order to do this you need to rebuild PHP with the following change in the imagepsextendfont function
in ext/gd/gd.c:
/* ADD THIS: */
T1_DeleteAllSizes(*f_ind);
/** RIGHT BEFORE: **/
if (T1_ExtendFont(*f_ind, Z_DVAL_PP(ext)) != 0) RETURN_FALSE;
RETURN_TRUE;
That will allow you to call the function twice for the same font resource.
This has also been submitted as bug #14735 at http://bugs.php.net
