I believe require("/includes/myinclude.php3") fails where require($DOCUMENT_ROOT .
"/includes/myinclude.php3") succeeds is because php is running in the unix environment,
not the apache environment, meaning that '/includes/myinclude.php3' is a call relative to
the root directory of the filesystem.
This is a guess, but I wouldn't be surprised.
