I just wasted half a day working this out...
If $user_file repeated contains 'none', make sure the line in the form
<input type="hidden" name="MAX_FILE_SIZE" value="1000">
has a value greater than 1000! Otherwise you can only upload files smaller that 1000 bytes!!
