I have the similar problem, after installing 10.1.5 PhpPgAdmin isn't anymore working correct. I
can open the database and view the table structure, but it is not possible to view the records. I
only het an errow message:
Failed opening " for inclusion .. in /Library/WebServers/Documents/phpPgAdmin/sql.php on line
