I`ve read all that You say and try to do next two simple files. First file make include throught Web: <?php include('http://YourTempAddress/test.php'); ?> Second File (test.php) make output as html, but result is the new php script: <? echo "<?\$ip=getenv('REMOTE_HOST');echo \"IP=\$ip;\";?>"; ?> That work perfectly without searching of server that does not support of php scripts but allow to store php files on it. Extention is also not important. So that is more secure to reject using of including through Web.
