Using the directive
AddType application/x-httpd-php .mdt .mdtc
if you are using header.php and footer.php included in index.mdt, the web server will not treat it
as a php file, because it's not in the AddType listing.
Either rename header.php to header.mdt (same with footer) or change your AddType line to:
AddType application/x-httpd-php .mdt .mdtc .php
and just don't use .php on any public files.
