Notes will now not appear in the manual until a notes editor approves
them. When the changes take place (30 minutes from now, probably),
there will be an 'Approve Note' link in the emails sent to php-notes.
If the note is worthy of a place in the manual, approve it. All other
functions (edit/delete/reject) still work the same.
