note 33089 added to ref.yaz

Like every newbie I was getting the ".\php_yaz.dll" not found error. Then you realize that you have to copy it from the extensions directory into the System32 directory as well as enable the extension in the php.ini file. However, I have found that just doing the above still didn't work on my Apache server. I downloaded the Yaz toolkit from http://www.indexdata.com (the brains behind this generously free z39 implementation) and installed it. I copied the Yaz.dll (different from the php_yaz.dll) into System32 and yahoo! It worked!
