note 34216 added to ref.mssql

To use the entire API, be sure to compile php with the --with-mssql switch and *not* the --with-sybase switch. Some MSSQL functions will work with the sybase switch, but not all (case in point, mssql_field_type()). One other minor note: be sure to include the port number when specifying the IP address of the MSSQL server. It solved my connectivity problem with the phplib (7.4-pre2) database tools.
