If you plan to use the PHP_DOCUMENT_ROOT environment variable to dynamically set the document root,
make sure that the doc_root configuration directive is commented out of your php.ini file. If set,
it will override PHP_DOCUMENT_ROOT, and your environment variable will be ignored in favour of the
one in the file. This is contrary to the impression I got from the documentation.
