If you want your page not to be cached, be careful with this:
header("Last-Modified: " . gmdate("D, d M Y H:i:s") . " GMT");
// always modified
In case the server time is wrong (in the past), browsers may cache the page and even Reload
doesn't help. The problem appeared when I upgraded to Apache 2 + PHP 4.3.1 (Linux).
