I spent hours trying to connect to a remote SQL server. I was able to connect fine using ODBC but
not using the mssql_ library.
The trick to making this work is not only installing the client component of SQL on the PHP server
but also using the Client Network Utility to (a) set the default protocol to TCP/IP (b) set up an
Alias to the remote hostname/ip address.
