Regarding the DTD parsing problem, I came up with:
<?php echo('<?xml version=\"1.0\"
encoding=\"UTF-8\"?>\n'); ?>
Using ?> instead of ?> prevents the parser from interpreting it as a closing tag; but it
still reads as ?> in HTML.
