In Windows/IIS, if you're getting an access denied: php_mcrypt.dll error, you need to make sure
that:
1) the IIS user (IUSR_[WEBSERVER]) has read and execute permissions for php_mcrypt.dll
2) the php extensions directory is properly specified in php.ini (extension_dir=
"c:\php\extensions" or what-have-you)
3) the IIS user (IUSR_[WEBSERVER]) has read and execute permissions for
/WINNT/system32/libmcrypt.dll
