The html_entity_decode function submitted by dave at codexweb dot co dot za for PHP < 4.3.0 users
doesn't handle the simple quote correctly.
The following line
$trans_tbl["'"] = "'";
should be replaced by
$trans_tbl["&039;"] = "'";
