note 45888 added to install.problems

Installing php5 on IIS6/Windows 2003: After following installation instructions ISAPI module still won't work. I don't know for other configurations of IIS, PHP and Windows, but on this particular combination, one must grant privileges to the files (and DLLs) used by PHP to user IUSR_serverName where serverName is your server's name. I first heard about INET_USR_serverName, but that name was not existent on my server, it used IUSR_serverName.
