note 46946 added to ref.mssql

This note is for mssql under a Windows environment. If you are having problems connecting to MS SQL using the mssql PHP extension then I would suggest that you get a copy of the latest NTWDBLIB.DLL. Copy it to the directory where PHP expects to find it's dll files.
