There is one small error in the code given by dmurphy:
The replacement for "&" must be the very first in the array.
Currently, the & of """ is replaced by the & entity, which leads in
effect to "
"34"=>""", "38"=>"&"
Just move the "38"=>"&" to the first position in the array.
