my ISP does not have upload_tmp_dir defined; my uploaded files were landing in /tmp. for whatever
reason, my call to move_uploaded_file() was failing w/ an error about not being able to open the
stream.
the fix was to redefine upload_tmp_dir by adding this line to the .htaccess in my public_html:
php_value upload_tmp_dir /home/zim/public_html/upload
