On Windows 2003 Server and IIS as web server you must do one additional step!
In IIS Manager open Web Service Extensions click on the Add a new Web service extension... link.
Type extension name example: PHP, and Add as a Required file php4isapi.dll or php5isapi.dll and
enable the option set extension status Allowed otherwise you will become a 404 Error page.
