note 58882 added to features.file-upload

I'm very confussed here, I can paypal 5$ to whoever can tell me exactly waht to copy and paste into the page, and the php file it connects too. I want it to be able to upload 5 timeases at a time(5 bwrase fields, 1 upload) images only, i want it to be validated, any extension is fine, gif,jpg,jpeh,bmp,psd, any IMAGE exetion is file that is, MAX upload size is 5MB's.
