Hard to believe that it wasn't installed, as QuickForm will not work
without HTML_Common at all.
There is a dependency for HTML_Common 1.2.1, but PEAR installer tends
to incorrectly process it. Please try the most recent 'PEAR' package
and submit a bug for it if it does not work as expected.
