Currently I'm running a local mirror, so I only install/upgrade the packages we need. The package is first tested on a test machine and if no weird things are found, it's added to the local mirror. Our webservers synchronize every night with the local mirror, almost in the same way we manage our Debian machines, which also upgrade at night. All our PHP sites have a test page, which includes all the packages used by a site and are than tested. The results are mailed to the web administrators. These verify if every thing works as meant. So, this option would help us manage the pear install over the various webservers. Currently it's done by a script, which download all packages and extract the package.xml file and validates it with the local mirror to check if it's an update and if the state is 'stable'.
