Bug description: Warning breaking bug system

Every time something is done (submitting a report, closing, etc.), I get a
warning (see below) which stops the rest of the page from showing up.
When you hit refresh, it tries to do it again, causing duplicate bugs.
Expected result:

A confirmation that the action has happened.
