<W3C:Fanatic> <em> means Emphasis, <strong> means Strong Emphasis - the fact they are styled in italics and bold respectively is a coincidence. Theres nothing in the specification to say that a useragent should render them these way, they are for purely semantic meanings - as all (X)HTML tags should be.
I completely agree, and share your enthusiasm for semantic markup rather than physical markup (c.f. the deprecation of bold and italic and newlines in Text_Wiki). This case, however, has a mitigating factor. The [i] binding in BBCode and its translation to <em> in this particular case would lead one to believe that BBCode figures it for italics. Also, many if not all browsers default to rendering <em> in a fashion similar to <i>, so there is a user expectation to deal with as well. None of this argues against the W3C standard, only my own (and perhaps others') expectations as to how <em> should render in this case.
