I've also discussed "porting" FPDF to PEAR with Olivier (the author of the library), but he seemed rather not interested.
Also, FPDF can become easily bloated by all the add-ons, so I would recommend some type of decorator design-pattern for this class. You guys can probably think about a solution for that.
Also, the source code of FPDF would need a lot of cleaning up to be brought up to PEAR standard...
Otherwise, I am currently trying to do some kind of XHTML+CSS to PDF converter based on FPDF... it is quite complicated though...
