Have you read http://pear.php.net/manual/contributing-requirements.php?
In general, PEAR is open to C and PHP code, so there is no problem
to have your extension. Have a look at XMMS (Rasmus commited that
some time ago): This is an example how C module are integrated
in PEAR.
