I believe what you're looking for is the "install as" option in package.xml v2.
<phprelease>
<filelist>
<install as="mdb2_schematool/action.php"
name="www/mdb2_schematool/action.php" />
<install as="mdb2_schematool/class.inc.php"
name="www/mdb2_schematool/class.inc.php" />
<install as="mdb2_schematool/index.php"
name="www/mdb2_schematool/index.php" />
<install as="mdb2_schematool/result.php"
name="www/mdb2_schematool/result.php" />
</filelist>
</phprelease>
In PEAR_PackageFileManager2 you can use:
$pkg->addInstallAs('www/mdb2_schematool/action.php',
'mdb2_schematool/action.php');
...
