You can always introduce it. It's true that things has changed but the
only things you would need to add pearinfo are to create a
Frontend/Web.php and a Commands/Info.php files inside php4/pear/PEAR/.
For the pear_base_install, you can always set it on-the-fly:
$ pear -d php_dir=foo install <package>
or make the change in the default configuration persist:
$ pear -d php_dir=foo -s
$ pear install <package>
