Abiword's DocBook is sort of OK, but sometimes it does some funny things with entities. In the PHPDOC (the PHP manual documentation team) we just use VI/VIM, Emacs or any other text editor, and just have some empty templates we use to start a new document. I find that VIM 6.x is plenty for dealing w/ DocBook, with some simple functions for making tag generation simpler (which I can put in a tarball if someone is inerested, when the OSCON finishes), similarly the SGML mode of emacs seems to work OK but I had not tried myself.
