I got an idea: Let´s add a new parameter to setFetchMode()! If set, a
new boolean property is set that can be checked for in getAssoc(),
resulting in a change of behaviour if necessary.
We could call this parameter something like $evenForGetAssoc or
$iReallyReallyMeanIt :-)
