Hello,
I had a first check on your report and the example at
http://pear.php.net/manual/en/package.html.html-
template-flexy.html-template-flexy.html-template-
flexy.php
looks accurate to me.
I am sure that the package lead developer will be in the
position to further comment asap.

Description:
------------
On the first page of the Flexy documentation there's an example in
which you instantiate a new HTML_Template_Flexy_Element.
However, it does not work until a
require_once 'HTML/Template/Flexy/Element.php'
Reproduce code:
---------------
$this->elements['input'] = new HTML_Template_Flexy_Element;
Expected result:
----------------
new HTML_Template_Flexy_Element be created and working example
Actual result:
--------------
Fatal error: Cannot instantiate non-existent class:
html_template_flexy_element in e:\www\flexytest\index.php on line 45
