The template you posted before contained placeholders for elements 'requiredFields', 'page', 'comment1' and 'submit'. No such elements are defined in this script. I suggest you review the examples on ArraySmarty that come with QuickForm package and try to use the renderer without HTML_QuickForm_Controller first. The info in the QF_Controller manual *is* short precisely because you are expected to read QuickForm docs / try its examples as well. No use in repeating.
