HTML_QuickForm_hierselect issues/suggestions

Issue 1: I'm playing around with hierselect and I tried using
associative arrays for content, as that would seem to provide me with
more useful data coming in from the form rather than having numerical
indexes (that way I don't have to remember and lookup what 0 or 3
whatever match up to...). When I do, it seems to work correctly and
submit the right data, except I get warnings:

The array looks something like this:
$select['cc'] = 'Test CC';
$select['bb'] = 'Test BB';
.
.
.
$select['cc']['test'] = "Testing";
$select['cc']['test2'] = "Testing2";
.
.
.
Issue 2: Sometimes it may not be necessary to use all the levels of the
select hierarchy. As it is now, all the selects are there all the time,
even if they're totally empty. It might be helpful if they could
disappear, or at least be disabled or something to indicate that there
are indeed no choices available.
