Try this:
PEAR::setErrorHandling(PEAR_ERROR_DIE);
$dl = &new HTTP_Download();
$dl->setFile('test.doc');
$dl->setContentDisposition(HTTP_DOWNLOAD_INLINE, 'test.doc');
$dl->setContentType('application/msword');
$dl->send();
