If you're looking for a form based on the database look no further than DB_DataObject_FormBuilder. It will auto-generate forms based on your database. You *do* have to set up DB_DataObject first, but it's not too hard. If you're having troubles, feel free to contact the list (or me). I've also added support for the checkbox thing you mentioned. Well, actually, Norbert Mocsnik created the code, but I've prettied it up a bit.
