Urgh! The answer was so simple.
All internal urls in the $menu data provided must begin with "/" in order to
be mapped from the root directory. I changed my urls from (for example)
"index.htm" to "/index.htm" and all my problems vanished.
No need to forceURLs or add prefixes.
Too easy.
