If you are getting code from CVS, you need to install it:
cd HTML_QuickForm
pear package
(... a .tgz file will be generated ...)
pear install filename.tgz
or you could just do:
pear install HTML_QuickForm
and the installer will pick the latest release from pear.php.net
I think this info is in the PEAR manual, but I might be wrong.
