Problems with DB_DataObject_FormBuilder example

Hi, I've been trying to get the example that comes with DB_DataObject_FormBuilder (DBDOFB) to work to give me an idea of what the package is capable of. However I have not been able to get this example to run; I tried rebuilding the contents of the 'classes' which improved matters a little, but I still get no form displayed. Turning debugging up to 7 (well, I had to try something) gives a lot of stuff.
