Doug,
I just started playing with HTML_QuickForm myself so my advice may not be that good but I believe that if you want to control the output of columns in your table you should take a look at the example in the documentation in your local pear installation.
/pear/docs/HTML_QuickForm/docs/renderers/QuickHtml_example.php
It produces a table with the colspan attribute applied.

$form->addElement <http://www.php.net/manual-lookup.php?lang=en&pattern=addElement>('select', 'mAnonymous', 'Make this an Anonymous request?: ', $yesno, $attr);
?>
outputs :
<tr>
<td align="right" valign="top"><b>Make this an Anonymous request?: </b></td>
<td valign="top" align="left"><select colspan="2" name="mAnonymous">
<option value="0" selected="selected">No</option>
<option value="1">Yes</option>
</select></td>
</tr>
instead of
<tr>
<td align="right" valign="top"><b>Make this an Anonymous request?: </b></td>
<td valign="top" align="left" colspan=2>
<select name="mAnonymous">
<option value="0" selected="selected">No</option>
<option value="1">Yes</option>
</select></td>
</tr>
How do I correctly generate the later statement?
Regards,
Doug
